Survey (downloadable form) New hospital in Cumbria
Cumbria PCT is keen to learn from the experiences of those who have received health and social care services within care homes or within their own homes. They are interested in your views and opinions so that they can take these into account when planning new services and reviewing how existing services might be able to improve.
We would be grateful if you could take some time to complete this questionnaire if you receive health or social care service either at home or in a care home or if you are a family member of someone in receipt of care or if you have professional knowledge in this area.
